Notice to Uplift 8990 Holdings, Inc. (HOUSE) Delisted Shares

Relative to the voluntary delisting of 8990 Holdings, Inc. (HOUSE) effective October 31, 2025, PDTC shall cease to recognize the eligibility of all securities which have been delisted by PSEI.

In this regard, clients are requested to convert the HOUSE scripless shares into certificated shares under your name.

Please submit the documentary requirements listed below not later than November 12, 2025, at 5:00 pm, otherwise, the shares will be registered under the name of BPI Securities Corporation FAO (client name).

1. Upliftment Request Form

2. Specimen Signature Card & Additional Personal Information

3. Photocopy of 2 valid government issued ID’s

Kindly take note that we will debit the Upliftment Fee of PHP200.00 from your nominated BPI Trade Settlement Bank Account (SETA).
