Notice to Uplift Premium Leisure Corp. (PLC) Delisted Shares

Relative to the delisting of Premium Leisure Corp. (PLC) effective July 9, 2024, PDTC shall cease to recognize the eligibility of all securities which have been delisted by PSEI. In this regard, you are requested to convert the PLC scripless shares into certificated shares under your name.

Please submit the documentary requirements listed below not later than July 31, 2024, at 5:00 pm, otherwise the shares will be registered under the name of BPI Securities Corporation FAO client name.

2. Specimen Signature Card
3. Additional Personal Information
4. Photocopy of 2 valid government issued ID’s
5. Bank Certification verified by bank officer, or you may visit the office of BDO Stock Transfer for personal appearance located at 15/F BDO Corporate Ctr. Makati Ave. Cor H.V. Dela Costa St., Makati City
